    Can’t Take Statins? New Pill Cuts Cholesterol, Heart Attacks 


    In a major study, a different kind of cholesterol-lowering drug named Nexletol reduced the risk of heart attacks and some other cardiovascular problems in people who can’t tolerate statins, researchers reported Saturday. Doctors already prescribe the drug, known chemically as bempedoic acid, to be used together with a statin to help certain high-risk patients further lower their cholesterol. The new study tested Nexletol without the statin combination — and offers the first evidence that it also reduces the risk of cholesterol-caused health problems. (Neergaard, 3/4)

    After Its Drug Was Shown To Prevent Heart Attacks, What’s Next For Esperion?


    On Saturday a new study showed that Nexletol, the cholesterol-lowering medicine made by Esperion Therapeutics, prevented heart attacks among people who cannot or will not take potent cholesterol-lowering statins. The question now is whether those benefits are going to be enough to make sales of Nexletol take off. They have been basically dead in the water since the oral medicine was approved three years ago.     ‘Keto-Like’ Diet May Be Associated With Heart Disease, According To New Research


    A low-carb, high-fat “keto-like” diet may be linked to higher levels of “bad” cholesterol and double the risk of cardiovascular events such as blocked arteries, heart attacks and strokes, according to new research. “Our study found that regular consumption of a self-reported diet low in carbohydrates and high in fat was associated with increased levels of LDL cholesterol – or “bad” cholesterol – and a higher risk of heart disease,” lead study author Dr. Iulia Iatan with the Healthy Heart Program Prevention Clinic, St. Paul’s Hospital and University of British Columbia’s Centre for Heart Lung Innovation in Vancouver, Canada, said in a news release.     New Treatment Could Help Fix The Heart’s ‘Forgotten Valve’ 


    For the first time, patients with damaged tricuspid valves in their hearts might have a safe treatment that actually helps. More than 1 million mostly older Americans have seriously leaking tricuspids, a valve on the right side of the heart that lets deoxygenated blood flow between the right atrium and the right ventricle. When the valve leaks, blood flows backward. As a result, fluid accumulates in vital organs while legs and feet get swollen. The eventual outcome is heart failure. (Kolata, 3/4)

    Lesion Removed During Biden’s Physical Was Cancerous 


    President Biden had a cancerous lesion removed from his chest during his physical last month, the president’s doctor said Friday. The existence of the lesion was included in the summary of Mr. Biden’s physical at Walter Reed National Military Medical Center in mid-February. On Friday, Dr. Kevin C. O’Connor, the president’s longtime physician, said a biopsy confirmed that it was basal cell carcinoma, a common and relatively unaggressive form of skin cancer. (Rogers, 3/3)

    Just over two weeks ago, President Biden had skin cancer, but today, he doesn’t. According to a White House physician’s memo on Friday, doctors at Walter Reed National Military Medical Center removed a lesion off his chest on Feb. 16, treated the area around the tumor site, and that was that.     Washington, Oregon To End Health Care Settings Mask Mandate


    Washington and Oregon will soon drop mask requirements in health care settings, state health officials said Friday, moving to lift the last major masking requirements meant to curb the spread of COVID-19. Mandates in both states will end on April 3, meaning health care workers, patients and visitors will no longer be required to wear a mask in facilities including hospitals, urgent care centers and dental and doctors’ offices. Washington’s mask requirements in correctional facilities will also end April 3. (3/3)

    Five People Are Dead At A South Yarmouth Nursing Home After A COVID-19 Outbreak


    Five residents of a South Yarmouth nursing home died in recent days following an outbreak of COVID-19, which also caused more than 90 additional cases among residents and staff. The state Department of Public Health has ordered Pittsfield-based Integritus Healthcare, which operates Windsor Skilled Nursing and Rehabilitation Center in South Yarmouth, to cease admissions of new residents while officials respond to the outbreak.     Officials from the World Health Organization on Friday said that all COVID-19 origin theories remain viable despite recent U.S. reports promoting the idea that the deadly virus originated in a lab in Wuhan, China. “If any country has information about the origins of the pandemic, it is essential for that information to be shared with WHO and the international scientific community,” Director-General Tedros Adhanom Ghebreyesus told a press briefing.     Utah Governor Says He Plans To Sign Abortion Clinic Ban 


    Utah Gov. Spencer Cox said Friday that he plans to sign a measure that would effectively ban abortion clinics from operating in the state, meaning hospitals will soon be the only places where they can be provided in the state. After passing through the state Senate on Thursday with minor amendments, it returned to the Utah House of Representatives Friday morning, where it was approved and then sent to the governor for final approval. The move comes less than a year after the U.S. Supreme Court overturned the Roe v. Wade decision, returning the power to regulate abortions to states. (Metz, 3/4)

    Rite Aid Corp. said it was “monitoring the latest federal, state, legal and regulatory developments” and would keep evaluating its policies. The Associated Press also sought comment from CVS Health Corp., retail giant Walmart and the grocery chain Kroger. Some independent pharmacists would like to become certified to dispense the pills, said Andrea Pivarunas, a spokeswoman for the National Community Pharmacists Association.     Judith Heumann, a renowned activist who helped secure legislation protecting the rights of people with disabilities, has died. She was 75 years old. Ms. Heumann died Saturday in Washington, D.C., according to a statement from her family. Over decades of activism, Ms. Heumann played a role in developing national disability-rights legislation, including the Americans with Disabilities Act. She was also involved in the passage of the United Nations’ Convention on the Rights of Persons with Disabilities. (Otis, 3/5)

    Another Mississippi Hospital Will Stop Delivering Babies


    A hospital on the Mississippi Gulf Coast will suspend labor and delivery services April 1 because of a shortage of obstetricians, further decreasing health care access in a state that has seen other hospitals shut down birthing centers or intensive care for newborn babies. Singing River Gulfport said in its announcement Thursday that hospital leaders hope the suspension of services will be temporary, WXXV-TV reported. (3/3)

    Hospital systems are turning to cross-market mergers to satiate their thirst for growth and avoid antitrust heat. But hospitals are also still signing other lower-profile deals, which experts believe inevitably lead to higher insurance premiums and create more medical bill stress for people in those communities. “Smaller mergers just don’t get the attention they deserve because they affect fewer people,” said Vivian Ho, a health economist at Rice University who studies hospital consolidation.     Nevada Crash Is 3rd Fatal One Tied To Air Medical Service 


    The company that owns the medical transport aircraft that crashed in northern Nevada last week, killing all five people aboard, has been tied to two other fatal crashes in the last four years. A review of records shows that with the latest crash, 11 people total have now died on planes owned and operated by Guardian Flight, the Reno Gazette-Journal reported Friday. The company is also now facing its fourth National Transportation Safety Board probe since 2018, said Bruce Landsberg, NTSB vice chairman. (3/3)

    Eli Lilly Will Avoid Big Medicaid Rebates After It Cut Insulin Prices


    Eli Lilly would’ve had to pay Medicaid about $150 for each vial of insulin used in the program if it hadn’t dramatically cut the list prices for some of its older products this week. The company was about to run into a Medicaid penalty for hiking the price of its drugs faster than the rate of inflation. Now that it plans to lower the list price of the insulin Humalog 70{fe463f59fb70c5c01486843be1d66c13e664ed3ae921464fa884afebcc0ffe6c}, it won’t trigger that penalty.     The HPV vaccine is a slam dunk in preventing the vast majority of cancers related to the infection — namely tumors of the head and neck, anus, penis, vagina, and cervix. But that’s only for people who got shots early enough to prevent HPV infection. Everyone else must hope for other vaccines that scientists are developing to treat existing HPV-associated cancer.     Colorado Psychologists Eligible To Prescribe Medications Under New Law


    Psychologists in Colorado will be allowed to write prescriptions if they’re willing to obtain an additional two-year degree, under a new law signed Friday by Gov. Jared Polis. The measure is intended to increase access to mental health care in Colorado, which has a severe shortage of mental health professionals.     Florida Bills Would Ban Gender Studies, Limit Trans Pronouns, Erode Tenure 


    Florida legislators have proposed a spate of new laws that would reshape K-12 and higher education in the state, from requiring teachers to use pronouns matching children’s sex as assigned at birth to establishing a universal school choice voucher program. The half-dozen bills, filed by a cast of GOP state representatives and senators, come shortly before the launch of Florida’s legislative session Tuesday. Other proposals in the mix include eliminating college majors in gender studies, nixing diversity efforts at universities and job protections for tenured faculty, strengthening parents’ ability to veto K-12 class materials and extending a ban on teaching about gender and sexuality — from third grade up to eighth grade.     Legionnaires’ Disease Found In 2 Past Las Vegas Hotel Guests


    Las Vegas area health officials say Legionnaires’ disease was found in two people who stayed at the same hotel in recent months. The Southern Nevada Health District announced Friday it is looking into two cases reported in guests who stayed at The Orleans Hotel & Casino a few miles west of the Strip. One guest visited there in January. The other in December. The hotel is informing current and past guests going back to Dec. 16 of possible exposure. (3/3)

    Norovirus appears to be at a 12-month high, according to data from the Centers for Disease Control and Prevention.     A person in southwest Florida has died after being infected with an extremely rare brain-eating amoeba, health officials said. The Florida Department of Health in Charlotte County confirmed the death Thursday. The agency had previously issued an alert last month, warning residents about the Naegleria fowleri infection.     Regular Laxative Use Correlated With Higher Dementia Risk In U.K. Study 


    Regular laxative use may be correlated with dementia, according to research published in the journal Neurology in February. The study looked at a cohort of 502,229 British adults participating in UK Biobank, a long-term initiative that gathered extensive genetic and health information from 40- to 69-year-olds in England, Wales and Scotland between 2006 and 2010. The participants had no history of dementia.     All skin tests doctors commonly use to check for food allergies can provide false negative results, the Food and Drug Administration has concluded — meaning people with potentially life-threatening allergies could mistakenly be told they are not at risk.     CDC Warns Of Risk To Travelers From Chikungunya Outbreak In Paraguay


    The Centers for Disease Control and Prevention (CDC) yesterday issued a Health Alert Network (HAN) Health Advisory to clinicians and public health officials warning that US travelers could be affected by a growing chikungunya outbreak in Paraguay. Since the chikungunya outbreak began in October 2022, the Ministry of Health in Paraguay has reported 71,748 suspected cases of the mosquito-borne alphavirus, with 29,362 of those cases being probable or confirmed.     I don’t feel old on most days, but I’m definitely starting to feel things that I didn’t feel a few years ago. Things like a grouchy knee, IT band issues, sore muscles, tendonitis, and a few other ailments that let me know I’ve a got a few years under my belt. I’m certainly not ready to throw in the towel, but I’ve started coming to terms with the fact that I probably have more years behind me (I’m 45) than I have ahead of me…at least when it comes to the rough-and-tumble world of backpack hunting the wilderness and the grind of extreme mountain hunts. I bring this up because I’m currently at a hunting camp, guiding mule deer hunters, and there was a discussion last night at the dinner table about fitness, health, and the longevity one might have when referring to strenuous hunts and how you might extend those years.

    I’m sure some of you reading this are saying, “I’m 65 and still getting after it…you’re still a pup,” and I wouldn’t disagree. What I would say is that, at times anyway, I should have listened to the older crowd when I was about to do something physically stupid in my younger days. And even though I know better, I’m still abusing my body and making poor choices today. So why is that? Some of it might be bravado or ego, the internal struggle to prove to myself I can still run with my younger self — and maybe, just maybe, because I can still do it. The why doesn’t really matter, but what does matter is that I don’t injure myself while doing said stupid tasks. I want to stay in the mountains/wilderness as long as I can, and at this point I’m still running strong.

    So, with the thought-provoking conversation last night, I thought I would cover the good, the bad, and the ugly of what I’ve done to my body and what I’ve done to stay relatively healthy and injury free, and how I’ve altered my diet and workout program over time.

    Diet

    As boring a subject as it may be, dieting is probably the most important and most overlooked ingredient to a long lifespan in the field, as well as your day-to-day life. More than once I’ve told people that they should probably lose 40 pounds off their butt instead of worrying about dropping four ounces in pack weight. I speak from experience on this one! When it comes to gear, if you have money, it is easily attained without hard work or discipline. Dieting and nutrition demand a lot of discipline, hard work, and determination. Diet is 65 to 70 percent of the battle. In other words, YOU CAN’T WORK OUT.

    Enough To Outweigh A Bad Diet!

    Every body type is going to be a little different, and that means everyone’s diet will be different. However, cutting out processed carbs/sugars, gluten, and drinking 100 ounces (or more) of water a day is probably a safe path to a better and healthier life. This means giving up soda, cookies, crackers, chips, and all the other good stuff we love to eat. This will help you lose weight but will also help with inflammation, gut health, and multiple other physical ailments. You can find a lot of good information and some unbelievable health and fitness coaches in the outdoor industry on the Internet. Get to know their programs and philosophies and see if they match your needs and personality. My diet plan, much like my workout plan, is pretty boring: High protein, high fat, some carbs (the good ones), and keeping away from processed carbs. I pretty much live off wild game, and my wife cooks all organic, so that’s a big help, too.

    Another potential downfall I’ve found for myself is overeating and binge eating. I’m a recovering fat kid, and having something sweet in the house is the same as having alcohol in an alcoholic’s home. For whatever reason, I have the discipline to keep junk food out of the house, but lack the discipline to not eat it when it’s in the house. SO, KEEP IT OUT OF THE HOUSE!

    Finding healthy substitute food has been another lifesaver for me. Find something to snack on to keep cravings down that isn’t too bad for you like jerky, raw almonds, cheese sticks, fruit (some fruit anyway), and the occasional somewhat healthy protein bar.

    One thing about a good diet plan is that eating healthy food will eventually become second nature and not a chore, as it will be in the beginning. You will also feel about 100 times better, and that will hopefully carry you through the rough times.

    Exercise

    Much like dieting, exercising every day will eventually become second nature, and in a lot of cases, an addiction. The problem is that there are so many exercise programs, fitness coaches, and marketing hype, that it gets confusing. Every person is going to have different goals, body types and builds, so a workout program specific to YOU is your best bet. This isn’t a must but is definitely something that you will eventually want to do.




    In my case, I’ve gone from the far ends of each spectrum on workout and fitness programs from a 275-pound powerlifter to a 190-pound backpack hunter/hiker. In the end, I have landed somewhere in the middle, with my focus more on mountain hunting and endurance than overall strength and an average body weight of 210–215 pounds. That weight keeps me around 11 to 12 percent body fat and allows for decent speed and endurance on the mountain, but also keeps a decent amount of muscle mass on me and a little extra fat when needed.

    Once you get into an exercise routine, it’s important to do something every day, even when it may not be convenient.

    The workout program I’ve stuck with to maintain this “condition” is pretty boring, but it works for me. I do have a home gym, but I can do a modified version in any hunting camp or hotel. I focus on push-ups, air squats, abs, and lunges, and I use resistance bands for my arms/shoulders and back. I don’t lift overly heavy weights and focus on supersets (going from one exercise to another with moderate rest) and keeping my heart rate at the higher end of fat burning. I also do a lot of backpack cardio and mountain biking, and when the weather stinks, I live on the rower and Hitmill X to keep the cardio going.

    As far as the reps/weight go, here’s a few examples of a normal superset routine:


    • Air Squats: 30 squats x 4
    • Push-Ups: 30 push-ups x 30
    • Curls: 35/40 lbs. x 4 reps x 12
    • Crunches: 50 crunches x 4
    • Overhead triceps extensions: 45/50 lbs. x 15 reps x 4

    This obviously isn’t the only workout I do, or the only muscle groups I focus on, but something like this will kick my butt in short order — and I can do these exercises anywhere I want to.

    This is what works for me, but if you’re confused in the beginning, then get a good trainer (in person or virtual) to help get you on the right path. A qualified trainer will also help you work around your current physical ailments/injuries to keep you going while also preventing future problems.

    For example, Matt Chan (a CrossFit trainer) sat down with me in my office several months ago and basically told me backpack cardio wasn’t something I needed anymore. He went on to explain that I had been doing it for so long that my body had adapted to it and I needed to change things up a bit as a result.

    Mountain-biking was one of Matt’s suggestions, because it’s low-impact and would help save whatever I had left in my knees. So, I purchased a bike from my friends over at Yeti, got the necessary gear to get going, and just rode the bike for three months. I was worried I’d get my butt kicked on our first big scouting trip. I was very wrong, and I did better hiking with a heavy load than I had in years!

    Don’t be afraid of working out or let it overwhelm you. Baby steps at first, and then go from there. Like dieting, you will ultimately live a happier and healthier life all the way around.

    Supplements

    The general idea behind supplements is taking something other than food — amino acids, caffeine, Creatine, protein powders, vitamins/minerals, probiotics, herbals, etc. — to help your performance and physical wellness. At one time, I took about every supplement I could to help my performance and muscle growth. But in the end, I realized a good diet was all I truly needed. I’m not saying some supplements don’t work; I’m just saying that as a mountain hunter who doesn’t want to stack on unwanted mass, they weren’t for me. I just want to get to my destination easier and feel as good on Day 10 as I did on Day One.

    While I don’t want to go into too much depth on this subject, I do feel the need to address this fact: Since hitting 40, my focus has been on improving my gut health, joints, core strength, and endurance, so my daily supplementation looks much different from what it was when I was 28. Here’s an example of what I mean:

    Water isn’t a supplement; it’s far more important. The human body is made of 60-percent water, and it is alarming how little water most people drink each day. On average, you should be consuming 80-120 ounces of water each day.

    Overtraining & Ego

    As far as longevity on the mountain goes, overtraining and ego (internal or external) can do some serious damage to the longevity of your outdoor career. This is a heavily debated topic on social media. I see both sides, and I’m as guilty as anyone for overtraining, lifting heavy, packing out entire animals, and telling people to “suck it up” when they start complaining about pack weight or intense workouts. The other side is, I’m starting to feel pain in overused body parts more often than I’d like.

    Would this be happening if I’d trained with less weight and frequency in the gym? Would my knees feel better if I hadn’t let my internal ego take over and pack out way more weight than I should have? Or would I be worse off by not hitting the gym and strengthening and conditioning my body? To these questions, I would say that like most things in life, a happy medium is probably the best approach. If you’re “that guy,” crank it up if it helps push you. I’m not saying train with medium weight or medium intensity, but you should definitely let your body heal and only go full-bore when necessary.

    I hope I’m making some sense, as I don’t want to be taken out of context, so if the time comes that packing out the entire animal is easier than dragging it, then get to packing. On the same subject, frequently training with 100-pound pack weights is a bad idea. I’m not saying you shouldn’t test the waters and see how you do with that much weight, but you are potentially taking years of life off your knees and back, so it should be done in moderation. At this point, I wouldn’t change anything I’ve done when it comes to training because it has been one hell of a ride, both in the gym, field, or wilderness. The question is, will I have the same outlook in 10 years? I’m guessing if my knees and body are so shot that I can no longer hunt, I will be saying something totally different.

    So, get your butt in the gym and train like a madman! You want to be strong, fit, and ready, but stay reserved until the time comes when you really have to get after it.

    Preventive Maintenance/Stretching

    Like most people I’ve talked to in my age bracket, I’m horrible about stretching and preventative maintenance — at least I was for the first 40 years of my life. I now own multiple foam rollers, Vudu floss and massage guns, and I see a massage therapist and dry needle specialist frequently. I’m not sure how much any of this would have helped when I was younger, but the aforementioned have mostly been a lifesaver for me in recent years. Stretching and maintenance isn’t always what I’d call “fun,” but speaking from personal experience, I will say it’s worth adding to your daily routine…and stick with it if it works for you like it has for me.

    You can search out a lot of info about the items I’ve listed, specific stretches for specific issues, and which exercises to do for combating current problems. I don’t want to go into too much detail with this as I’m not remotely qualified, but I would suggest, especially as you get older, finding someone who is, and they will most certainly help you out in many ways.

    Summary

    As I type this article, I’m sitting at a table in West Texas with aoudad and mule deer hunters I’ve been guiding. My knees hurt — especially my right one — my back is a little stiff, and both my shoulders are popping a little. I still do my morning workout each day and go as hard as I can on the mountain, but it wouldn’t hurt my feelings to have a little less pain at times.

    All the physical issues are daily reminders that I’ve been very blessed to experience some incredible adventures with amazing people. I try to keep that in mind when my body aches a little.

    A life-changing condition called POTS, which can cause fainting, irregular heartbeats and dizziness, particularly among young women, appears to be on the rise as a result of the coronavirus pandemic.

    But the condition isn’t well understood, and doctors dismiss many patients as having anxiety, delaying diagnosis. Once diagnosed, many patients face waiting lists as long as two years to get treatment from specialists.

    POTS stands for postural orthostatic tachycardia syndrome, a disorder of the autonomic nervous system, which regulates involuntary functions like heart rate, blood pressure and digestion.

    “When the autonomic nervous system is not functioning properly, any or all of those things can go a little haywire,” said David R. Fries, a cardiologist and POTS specialist at Rochester Regional Health.

    POTS patients typically experience a marked rise in heart rate when standing and a complex combination of symptoms, including dizziness, brain fog, fainting, headache and fatigue, among many others.

    There is no known cure for POTS, but physical therapy, medications and diet changes related to salt intake can sometimes help.

    Experts say there is a dire shortage of medical professionals who know how to care for patients with POTS. Lauren Stiles, president and chief executive of Dysautonomia International, a nonprofit advocacy group, estimates that the number of people with POTS has at least doubled since the start of the pandemic, while the number of specialists has remained the same and waiting lists are getting longer.

    “They were overwhelmed and flooded long before covid,” Stiles said “We need to increase the amount of experts in this because it wasn’t enough before covid, and it’s certainly not enough now.”

    POTS symptoms are often diagnosed as anxiety

    Symptoms can vary widely, and in some cases, can be debilitating.

    Angelica Baez, 25, of Sacramento, got sick with covid in March 2020. Soon after, she developed unusual symptoms, including tingling hands, frequent diarrhea and either feeling extremely cold or extremely hot. But the most debilitating symptom was that she almost constantly felt dizzy. She often felt like she might pass out and had trouble walking long distances.

    “My heart started going insane. I could feel it pounding in my chest,” she recalled. “I literally felt like I was going to die.”

    Just standing up or walking across the kitchen caused her heart rate to spike to 160 beats per minute, and she frequently rushed to the E.R., worried that she was having a heart attack. She tried consulting various doctors but was dismissed because nothing seemed wrong with her heart or overall health.

    “I would go to the doctor, and he just started telling me, ‘No, I think you just have anxiety,’” she said. “And I was like, you know what? I have anxiety because of what’s happening to me.”

    Baez was finally seen by a cardiologist who diagnosed POTS in early 2022.

    Life has changed dramatically for Baez since developing POTS. She needs to use a wheelchair to go longer distances and a shower chair to bathe. She gets dizzy if she sits upright or stands for long periods of time. Before getting sick, she was painting and interviewing for a job as a medical secretary. She can no longer work and can only paint in short bursts.

    Now, Baez is taking salt tablets and fluids and wears compression garments, which are both common interventions for POTS. She hasn’t been able to find a POTS specialist in her area. She sees a cardiologist and has been trying alternative treatments like acupuncture and herbal medicine.

    A 2-year-waiting list for POTS patients

    Part of the problem in diagnosing and treating POTS patients is that many doctors aren’t familiar with it and patients often see multiple doctors before finally getting diagnosed. One study of more than 4,800 POTS patients found that most didn’t get diagnosed until a year or more after first seeing a doctor, and the average diagnostic delay was nearly five years.

    It’s still unclear how many covid patients develop POTS. Studies show that anywhere from 2 percent to 14 percent of people with covid develop POTS. One study of long covid patients found that overall 30 percent met the criteria for POTS, although it was far more common in women. In that study, 36 percent of women met criteria for POTS compared to 7 percent of men.

    Before the pandemic, Tae Chung, director of the Johns Hopkins Postural Orthostatic Tachycardia Syndrome (POTS) program, said he had an appointment wait list that was about a year long, which was on par with many other POTS clinics. Now it’s a little over two years long, he said.

    Fries, the cardiologist and POTS specialist at Rochester Regional Health, said most cardiology appointments take about 40 minutes, but because of the complexity of symptoms associated with POTS, most of his appointments with POTS patients take 60 to 80 minutes. Because his caseload of POTS patients has roughly doubled since the pandemic, leading to longer wait times, Fries has started working on Saturdays.

    After Jillian York, 17, of Spring, Tex., got covid in June 2022, she experienced stomach aches and excessive sweating, and began having trouble sleeping. She felt dizzy after standing up and like she might faint when she performed as a drum major in her marching band. Getting up from the sofa caused her to blackout for a few seconds.

    Her mother, Heather York, 49, said she initially thought the Texas heat was to blame, but it soon became apparent that something was off.

    She started taking her daughter to doctors, but felt dismissed. Doctors attributed Jillian’s symptoms to her recent weight loss, despite the fact that she remained at a healthy weight.

    Jillian said doctors made her feel “minimized.” “I just felt really lost,” she said.

    Finally, in November, her mother took her to a pediatric neurologist, who told Jillian that she had POTS.

    Jillian has struggled to find a treatment that works. She has seen numerous cardiologists and other specialists and tried alternative interventions like acupuncture. Physical therapy has offered some relief, she said.

    “The lack of answers is the most frustrating thing that I have to go through,” Jillian said. “I know that more and more people are getting it, so why is it not being talked about?”

    At least a million new patients

    While nobody knows exactly what causes POTS, symptoms commonly develop following a concussion or viral illness, said Satish Raj, a cardiac sciences professor and POTS expert at the Libin Cardiovascular Institute at the University of Calgary. Most POTS patients are women.

    “There’s an element of dismissiveness and misogyny in the room. The POTS demographic is women who, for the most part, look pretty well,” Raj said. “They complain that their heart is racing, and I think that gets dismissed as anxiety a lot.”

    POTS wasn’t widely recognized as a real condition by the medical industry until a little more than two decades ago. In the United States, the condition only received a distinct diagnostic code in October 2022. Without a diagnostic code, POTS often got lumped in with other heart conditions, which is why it’s been difficult to track the overall number of POTS patients.

    Before covid, experts estimated there were about 1 to 3 million Americans who had POTS. And now there are at least 1 million or more new POTS patients as a result of covid, said Pam R. Taub, a cardiologist and professor of medicine at the University of California at San Diego School of Medicine, who is researching post-covid POTS for National Institutes of Health’s Recover covid initiative.

    Another barrier is that even after POTS was recognized, there historically has been little funding from organizations like the National Institutes of Health to research POTS or any potential cure. One analysis found that the NIH allocated, on average, $1.5 million dollars for POTS research funding per year between 2015 and 2020, whereas other diseases that are about as common in women as POTS, like multiple sclerosis and lupus, have received an annual average funding of $118 million or $127 million respectively.

    Potential treatments for POTS

    POTS patients often have low blood volume and difficulty with blood vessel constriction, so their blood tends to pool below their heart when they stand up. This depletes vital blood flow to the brain and can result in dizziness, brain fog, or fainting.

    Ingesting more salt and drinking more water may help people with POTS retain more fluid and increase their blood volume, experts say.

    In a small, controlled Vanderbilt University study, researchers found that a high salt diet improved but didn’t eliminate many symptoms of POTS.

    How much salt and water an individual with POTS needs can vary, but a high-salt diet may not always be appropriate for patients with cardiac complications from covid or high blood pressure, according to Svetlana Blitshteyn, an associate neurology professor at the University of Buffalo who is the director and founder of the Dysautonomia Clinic, which specializes in treating POTS patients.

    Other interventions include beta blockers or other drugs that can decrease heart rate. Medications to help the body better absorb salt and fluid, or blood vessel constriction medication, which allows the body to more easily get blood back to the heart and brain, are also used, she said.

    Physical therapy can also help many POTS patients, according to Thomas Chelimsky, neurology professor and director of the Comprehensive Autonomic Program and Autonomic Laboratory at Virginia Commonwealth University.

    Because POTS patients are sensitive to the effects of gravity, he typically has his patients start exercising from a seated or lying down position, or while in a pool.

    With treatment, Maura White, 44, of Rochester, N.Y., has significantly improved. She was diagnosed with POTS in October after getting sick for a second time with covid in April 2022. Before she got help, she was fainting about once a week and had a fluctuating heart rate.

    “I met with my general practitioner four, five or six times: each time going in crying and saying, ‘I’m not getting better, I’m not getting better.’ She would just say, ‘You’re going to get better,’” White recalled.

    It wasn’t until White went to physical therapy that she found out she might have POTS. White was finally diagnosed by a cardiologist in October and started taking beta blockers along with wearing compression stockings, eating more salt and doing physical therapy.

    She hasn’t fainted since starting the treatments. Although she used to be an avid runner, she isn’t able to stand for long stretches of time. Now she sits on a bench to shower, relies on grocery pickup so she doesn’t have to walk store aisles and rents a scooter for longer outings. She also sleeps about 12 hours a night and continues to have other symptoms like tingling hands and feet, muscle twitches, diarrhea and brain fog.

    Worsening symptoms after covid

    In some cases, covid seems to be worsening symptoms for existing POTS patients. Hanna Rutter Gully, 32, of Brooklyn developed POTS at the age of 17 after a concussion while playing soccer. She wasn’t diagnosed until six and a half years after the injury.

    Her symptoms, which included dizziness, migraines, cognitive issues and fatigue, made it difficult for her to attend college. She took an extra three semesters to graduate, finished many semesters remotely and had her sister move in with her in her final year of college to help her.

    Once she got diagnosed and got treatment, she was able to work her way up to living independently in New York and working full time as a corporate partnerships manager for Girls Who Code. Then, she got sick with covid in spring 2020 and her symptoms worsened to the point where she had to stop working.

    “You name it, and it’s gotten worse,” she said.

    Pre-covid POTS patients are having trouble getting appointments with specialists as well. Cara McGowan, 37, of Deerfield Ill., leads an Illinois support group for POTS patients. She has noticed that people have started to become more desperate for help. Some have lost their jobs as a result of their inability to get treated, she said.

    “The patient community is in crisis,” she said. “If you can’t take care of your health, everything else falls apart too.”

    Patients supporting other patients

    In many cases, patients are learning more about POTS from each other than they are from doctors, said Charlie McCone, 33 of San Francisco.

    McCone was a tennis, running and cycling enthusiast before he got sick with covid in March 2020. Then, he developed shortness of breath, chest pain, full body twitches, a rapid heart beat and fatigue, among other symptoms.

    After a second covid infection in August 2021, his symptoms worsened to the point where he had to stop working in his nonprofit marketing job. He couldn’t stand for more than five minutes or sit upright for more than 10 minutes without experiencing symptoms.

    He was not taken seriously by doctors, he said, and one doctor laughed at him when he asked whether his symptoms could be related to his coronavirus infections.

    It was only through a long covid support group that he figured out others were having the same issues. After seeing three cardiologists and two neurologists, he was finally referred to Stanford’s Autonomic Disorders Program and got diagnosed with POTS in May 2021.

    With treatment like compression socks and increased salt and fluid, he can now tolerate sitting up for 20 minutes before symptoms begin but still has trouble leaving the house.

    “It’s so frustrating that we are three years into this and long covid patients are having an incredibly difficult time getting a diagnosis and having their symptoms validated,” he said.
